Your profile states that you're caring for someone at home, and it also states that you would love to take care of someone and that you feel it's your "calling."

So, are you asking who would pay for care provided to the person living at home or in a situation in which you would be employed to care for someone?

If I were a home-based caregiver, I would say the best situation is to have a nursing assistant degree, and then work for an agency. The degree increases your hourly pay, and your inflow of clients. You can also work for home health agencies as a companion without a degree, and build your good reputation based on your success with clients. Most of your jobs may be private pay, meaning the government is not involved. Money comes from the retirement savings the elderly person has invested, or from family members who help pay for them. That is the largest area of job opportunity right now if this is your calling.
