My mother has been in a nursing home for three years and she still owns her house. Her ID has her house address on it and I have not changed her address with other places such as the IRS. She is in her last days and I wanted to find out if anyone knows what address I am supposed to put on the death certificate. Would it be her house address or the nursing home address? I am concerned that putting the nursing home address will cause probate and other problems. My dad was alive until a year ago so that is why the house is still there. Still working on getting it in saleable condition.

No one ever asked our family for any information to place on the death certificates. It was all done for us and we received them about 2 weeks afterwards. We didn’t choose address or any other information

When my dad died the funeral home asked me to review the death certificate for accuracy so I assume they will do the same thing with my mother.

drained, for my parents the funeral home asked what address I wish to use.

Thus, when my Mom was in a nursing home, Dad was living in the house they shared, so that address was used when Mom passed.

When Dad sold the house and moved to senior living, all his bills/statements were sent to my house, so my address was used on his death certificate.

Mom's death certificate was mailed to the executor's home. All documents needed for the disbursal of her estate were sent to him. ONE address makes it run more smoothly.

But you may not get to choose. I know when FIL died we got his mail fwded to our house and by darn, 18 years later, we're STILL getting stuff that is mailed to him. 90% of it is garbage, but 10% is the info about some property he owned and we have to pay the taxes every year.

The funeral home that we use just handed the death certificates to me when my dad died since I arranged the funeral. I think I will use a P.O. box for the forwarding. Thanks for the heads up.

I prepaid Moms funeral so the Funeral Director had all Moms info. My Mom was in a NH and still had her house. Her house address is on her death certificate.

Thank you for your response. I will insist that my mom's house address be on the death certificate. She probably has only a couple of days left from what I am being told. She has been unresponsive for the last 10 days and I now have hospice involved with her care. It has been so sad seeing her like this. Thanks again.

My mom passed away this morning, she is finally at peace. I want to thank everyone for the responses I was given to a couple of questions I had. I haven't been on this forum for very long but what I have read of other people's experiences and answers I received were very helpful. Also the funeral home is honoring my wishes to use my mother's house address on the death certificate so that issue is resolved. Thanks again for all your support.

I am so sorry for your loss. May you have peace and be comforted as you traverse this new terrain of grief.
I used my Mom's home adress as she still had her home after her AL stay. It has worked out well so far.

I'm so sorry for the loss of your mom.

Use her home address. With her passing, the NH can close her account (and be paid any outstanding charges) and you don't have to feel like dealing with them or having them connected to her in any way.

Thank you for your condolences. I have decided to use her home address. I was told at the funeral home that this was appropriate.

if she is on medicade.....use the nursing home address. you dont want to mess with the govt,

Why? From a business standpoint, NH would want nothing more to do with her since she is deceased.

I just looked at my Dad's cert scan - on my desktop - and as estate Rep, I had my parents' residential (home) address listed. He passed in the hospital and that is listed as facility address.

As G&C of my mother, because she's been in the NH for 2+ years, that will be the address that I list on hers, when that time comes. She no longer owns any residential property so it's different than your mom's situation.

I'd strongly suggest that you ask the funeral home or do some online research; often, law firms will have blogs that provide these answers. Otherwise, consult an attorney.

I truly do not think that it's a huge issue, BUT it may be and I'm just not aware so Pls seek professional advice. I'd say the funeral home; they are very helpful.

Thank you for your input and your condolences. I have looked online for information and it is conflicting. I just returned from making funeral arrangements and explained the nursing home situation to them and they said that using the house address is appropriate so I am comfortable with my decision now.
