I think we need more information .

Do you live with her? Is there a caregiver contract that specifies what your hours and duties, and how much your salary will be?

When you contact her children, saying that she needs more help, what is their response ?

No I don't live w/ her, I have my own family I'm trying to raise & I live about 30 min. away fr her. I'm not paid I just help her out weekly w/ groceries, organizing meds, errands & Drs visits as needed. When I tell her kids what's going on, they try to convince her over the phone to get help, but that's about it.

Is your Grandmother refusing outside help such as paid caregivers to help her with daily things? Or maybe her children have been trying to convince her to move into safer elder friendly environment, and she won't move.

My parents are in their 90's and have always refused to move. It I had a crystal ball 7 years ago to have seen the future, I wouldn't have enabled them to a point where they felt they didn't need to move, why should they, I was doing a lot of things for them. Oh my gosh, how all that can snowball quickly.
