Apply for a work visa. What state do you wish to work in? Figure that out and then I think you have to find an employment sponsor.

FYI there is a long wait in most cases.

This is not an employment site or an immigration office. You can do caregiving in your own country. Find some elderly or sick people who need care in their homes.

Caregiving is in demand all over the world. You can do caregiving work anywhere. It doesn't have to be in the United States or western Europe.

Also, with the new administration taking office in January, any types of visa from African countries and many other places will be few and far between. You won't have any better a chance trying to get into a western European country either. Countries like France, Germany, The United Kingdom, the Republic of Ireland, Austria, Holland, Sweden, etc... are not taking people in anymore. Especially people with zero skills or financial resources. Importing domestic servants to be caregivers or nannies really isn't really a thing in most European countries anymore.

I feel for you, but honestly don't waste your time and whatever financial resources you have trying to get into the United States because you will be turned down very soon.

Find people who need caregiving in your own country. They are there too.

You can go to extended care facilities in your own area and apply for care job. While you do not currently have certification as a CNA (Certified Nursing Asst) you may find some facilites will help you get qualified or will train you in care, even if you start as kitchen help or housekeeping. A good and kind attitude will get you anywhere, and help is badly needed.
Good luck.
