What kind of hospital doesn't submit the claim to Medicare?

You need to look into this problem because emergency services are covered by Medicare. However, there are instances where patients are kept on an "observation" status, which means that they are not in the emergency room & they are not officially admitted to a unit, and apparently Medicare will not pay for that. Either the hospital has to change the status to "admitted" or still under emergency room care for Medicare to pay.

But I have never heard of a hospital not submitting the claim to Medicare. That is REALLY STRANGE.

Yes. There is a form you fill out and return it to your region's location. Find it on medicare.gov. Why wouldn't the hospital file the claim for you? Didn't you have your Medicare card with you? Medicare without other insurance will only cover 80% of the bill, and you will not be reimbursed for the 20%. Is that the part you paid?

I think you can. Google search reimbursement from Medicare. You'll probably need to file a form titled "Patient's Request for Medical Payment" along with an itemized bill to the Medicare contractor in your state

On Medicaid, you call your caseworker IMMEDIATELY and apply for reimbursement. You will need the bill and the details of care from the facility.
For Medicare: submit your own claim to Medicare using Form CMS-1490S to get paid back. Do not expect full coverage, because the MD probably charges a lot more than what Medicare allows.

Have you ever tried signing up for something like Humana? There are insurances out there that help you supplement where Medicare leaves off. Definitely shop around and maybe even ditch Medicare if possible because I've heard nightmares about them not covering anything and many people are complaining about Medicare
