Contact your town's social services department. If they do not want to take the items, call a local church. They may know people who are in need.

Our local council on aging accepts many products for distribution to those in need. They take incontinence products, walkers, canes, etc..

Many towns have a Senior Center or even a "Lending Closet"
Some communities have "Diaper Banks where they will accept incontinent supplies. This might also include UN open tubes of creams, ointments, gloves and wipes.
Depending on the supplies you could check your local church if they have a PADS program there may be homeless that need supplies.
Know that even un open places can not accept medications, syringes and controlled supplies. For things like that contact you local Police Department many have containers in the department where you can dispose of items and the police will make sure they are destroyed.
It is a shame that medications can not be redistributed to underserved/underinsured people in a safe manner.

There should be an "in home heath agency" near you that will take donations. Maybe Visiting Nurses.

On top of what has already been mentioned, I know that hospice will also take any unopened medical supplies too.
